Admin (Providers) can sign up for the PEFA App with provider access to allow them to check in a referral

Referrals are checked in within the bookings section in the dashboard when you first log in.


When the request has not been checked in it will be a faded green select the request to open it.

When you click on the name you will be asked to fill in the below information
 

Check the name, date of birth and gender are all correct.

Enter the postal address and select the relevant photo ID option.


  

Enter the details for the emergency contact.

Confirm the job details are correct if they are select the Yes checkbox.



If the job details are not correct select the No checkbox for the JobFit Systems phone number so we can update the job details for you.


 

 

Consent


The consent can be completed by the assessor or by the provider, the default structure is that the assessor will complete. Contact support to change this to the provider if this is your preference to save time


The consent process is as follows

  1. Enter the participant’s name.
  2. Get the participant to sign in the section provided.
  3. Repeat the steps for the witness confirmation.
